Voluntary Reporting System is established in order to facilitate collection of information on actual or potential safety deficiencies. The system ensures that relevant data on safety is reported, collected, stored, protected and disseminated.
Reports submitted here will be de-identified at the level of Administrator. It is assured that no punitive action will be taken against the reporter of a voluntary report unless the reported infringement relates to unlawful/criminal/deliberate/gross negligent unsafe actions committed by the reporter.
